THUNDERBIRD HIGH SCHOOL

ATHLETIC CLEARANCE INFORMATION

 

In order for a student to be allowed to participate in a tryout session, practice, or compete in any AIA athletic event the following requirements must be met:

 

  • Thunderbird Athletic Clearance Packet

The Athletic Clearance Packet, which can be picked up in the front office, must be completely filled out and turned into the athletic secretary.

    • A physical examination as of March 1st is required (the doctor’s signature must be included)
    • The physical form is also available online. Click here to view and print the physical form.
    • Both sides of the yellow paper must be filled out by a physician.
    • All areas (lines) MUST be completely filled out
    • The student’s name is required FOUR times on the yellow cards and ONE time on the white Consent for Emergency Care
    • Parent/Guardian signature is required SIX times on the yellow cards and ONE time on the white Consent for Emergency Care
    • A student signature is required TWO times
    • Insurance is required.  Private insurance must include the name of the company and the policy number.  School insurance may be purchased through the athletic office.
    • Although information seems redundant, it is imperative that all areas of each portion of the packet be completely filled out.

 

  • Informed Consent Warning Film Card
    • All students are required to watch the Informed Consent video and have a signature card on file in the athletic office.  Students only need to watch the film one time during their high school career.

 

  • Birth Certificate
    • A copy of the student’s birth certificate must be on file in the athletic office.  Students age 19 on or before September 1st are not eligible for sports.

 

  • Scholastic Eligibility
    • Student who transferred from another school within a year from the ending date of the sport(s) he/she wishes to participate in MUST file a 520 form with the athletic office at least three weeks prior to the beginning date of the sport(s) in which he/she wished to participate.
    • Students in their first through six semesters (freshman through junior years) must be enrolled in a minimum of five credit earning classes.
    • Students in their seventh and eighth semesters (senior year) must be enrolled in a minimum of four credit earning classes.
    • Students re required to earn a passing grade in each class in which they are enrolled.
      • Students will be monitored for eligibility weekly.  Week one is a pending week and week two is the failing week.  Students who do not improve their grade to passing by Thursday at 10:00 during the failing week will be ineligible to participate in competition for two weeks or until the grade is improved to a passing status and the teacher notifies the athletic office.

Please take care of this as early as possible.

It is very common for Thunderbird to clear between 200 -300 students for each sport season!
